What is tic?
Trauma-Induced Coagulopathy (TIC) is a significant medical concern characterized by a disruption in the body’s ability to form blood clots following severe traumatic injuries. This impairment can lead to excessive and persistent bleeding, often proving difficult to control. Affecting up to 25% of trauma patients, TIC is associated with a higher risk of mortality. Currently, there's no universally agreed-upon clinical presentation for TIC, highlighting the need for standardized scoring systems.
Clinical scoring systems for tic
Researchers involved in the TACTIC (Trans-Agency Consortium for Trauma-Induced Coagulopathy) initiative have proposed quantitative scoring systems to aid in the assessment of TIC severity. These scores, ranging from I to V, are designed to be determined by the attending trauma surgeon after initial mechanical hemorrhage control measures are implemented. The scoring system helps categorize the severity of coagulopathy and guides appropriate treatment strategies.
The significance of trauma and hemorrhage
Trauma remains a leading cause of death and disability worldwide, with hemorrhage (bleeding) being the dominant mechanism contributing to mortality within the first 24 hours. Understanding the underlying mechanisms of TIC is crucial for developing improved treatments and management approaches to mitigate this excessive bleeding and ultimately enhance survival rates following traumatic injury.
Tactic: a collaborative research effort
Recognizing the critical knowledge gaps surrounding TIC, the National Institutes of Health (NIH) established the TACTIC consortium. This collaborative effort brings together researchers from the NIH and the Department of Defense (DOD) to investigate various aspects of trauma-induced coagulopathy, encompassing both large-scale clinical research and fundamental laboratory investigations.
Current resuscitation strategies and their limitations
Current resuscitation strategies for severe hemorrhage primarily rely on a reactive approach, involving blood component transfusions based on signs of clinical deterioration or hemorrhagic shock. A proactive approach is hampered by the lack of effective tools for early detection of TIC. The current practice of “damage control resuscitation”, with a 1:1:1 ratio of red blood cells, platelets, and plasma, may not be effective against TIC and can lead to complications like acute respiratory distress syndrome (ARDS) without improving survival rates.
Research focus areas within tactic
TACTIC's research focuses on several key areas to better understand and address TIC. These include investigations into the mechanisms and extent of thrombin generation in TIC, the role of platelet dysfunction, dysregulation of fibrinolysis, and the development of novel clinical platforms for early TIC assessment. By addressing these critical knowledge gaps, TACTIC aims to improve outcomes for trauma patients experiencing coagulopathy.
